Geralt didn't flinch, it was nothing he hadn't heard hurled at him before, usually followed by a derisive laugh from the speaker. The stranger in the gym might not have even meant for him to hear the word.
Even when people had heard about Witcher Syndrome, they seemed to forget about the more acute senses, like hearing. So he pretends that the insult had just been an aside that the man behind him had meant to keep to himself, and continues his workout.

Witcher Syndrome is a disorder that causes genetic mutations to occur to those who develop it. While some symptoms could almost be considered advantageous, others are clearly detrimental to those affected. As with any disability, there are those with prejudices against those that are “other” or “different”.
Geralt has spent most of his life learning to protect himself from being hurt by those who won’t understand that his differences don’t make him a freak.
But, it will take someone with quite a lot of zest for life and self-love, to show Geralt how to be okay with himself, and to remind him that at the end of the day, he is still only human.
Someone like Jaskier.

At the feeling, Noah made a squeak sound before moving Dude away from him.
"What… what do you think you’re doing…!?" Noah almost yelled, still surprised yet unaware of the fact that his friend of fur was after the remaining pieces of the peanut butter that fell off his own snack.
Dude then licked Noah’s fingers, who were full of the peanut cream. This time around, Noah let him once he realized what was happening.
