It's the pilot, somewhere in the middle, deleted-scene style. Dean has been having realistic dreams about fucking Sam. Sam THINKS he's having a realistic dream about being fucked by Dean, way better than the realistic nightmares about his girlfriend. And it's okay because it's just a dream! He doesn't have to admit to his waking self that deep down he really wants this, and never will!! He can live in denial.
Oops, nope, it's a cursed object that called them there and they're fucking for real. Magical convuluted accidental dub-con, my beloved!!!
(Ah, Pilot Winchesters... Sam didn't cheat, because he didn't know he wasn't dreaming. But once he had a taste... well, the Woman In White knew it would just be a matter of time. 😏)
