It's a good day until it isn't. The 'how' isn't as important so much as it's known to everyone that Jean is going to pass away in the near-future (which is confirmed by a call to Dr. Aaron). The knowing is worthwhile though because it gives Jean have time to share final requests and thoughts with both Kevin and Jeremy.
The crucial part of this chapter to know (taking out as much of the emotion as I can about it) going forward is what Jean asks both of them to do for him afterward. For Jeremy, it's to raise their son well, to live his life fully, to not pretend he's okay when he's not, and to find love again. (Obviously, that last one does *not* go over well.)
For Kevin, it's to not just take care of Jeremy and Jackie but to do so without stopping, to stay even when he's scared or doubts himself, to do everything for them as Jean himself would've done. (The wording of it is actually kinda important for the story later so I can put just the quote in a comment if you'd like.) Of course, Kevin promises Jean everything, even if he isn't quite sure he totally gets it.
The chapter ends with them knowing there's an undetermined amount of time left with Jean and them all wanting as much of it as they can get, even if it's hard.

Sure thing! I don't mind at all. As hard as it can be at times, I'm finding the process more cathartic than anything so I'm thankful thus far. I cut out all the tags/description between the [...] markers so here's the dialogue so you can see the important-later wording on the quotes:
