I don't want to give away the plot but the request was essentially for some heavy angst pertaining to this episode and some unexpected consequences of inter-dimensional travel for Sam. Didn't think it was possible, but when I saw the prompt, my brain went to that "challenge: accepted" place that seems to write so many of my cruelest fics for me.

Summary
The Frayed 'verse is canon-divergent after the end of season 5. When he's trapped for centuries with two pissed off archangels and a younger brother he can't always protect, in a space where he can never be sure of what's real and what's not, coming back from Hell is really rough on Sam. After he finally finds his way out, alone and frightened, and makes his way back to Dean, Sam has to deal with hallucinations, lifetimes of guilt and trauma, and a whole host of sensory issues. The world is too loud too bright too rough too fast too much, and for Sam, just existing is a whole new struggle. But Dean is there, and they do what they can to make it work.
And thanks to my amazing beta, tarotgal, who always helps me figure out where I'm going in this 'verse. It wouldn't be what it is without you.

Unfortunately, Sam is so shaken from his time in Hell that he can't—won't?—even talk to Dean, and when natural disasters begin to occur as frequently as they did when the Devil was walking the Earth, Dean realizes his brother may have acquired some of the angel's powers without picking up the ability to control them. In order to prevent Sam from hurting anyone, Dean finds a house as far from anything as he can manage and settles there to wait until the day Sam gets better—or until they both get killed by Sam's grace.

The Cage was never meant to hold a human. It was meant for one archangel, not two, and definitely not a human soul. Little by little Sam's soul was being destroyed by the torture the Cage inflicted on it. Lucifer and Michael joined together and, with their combined grace, pieced together Sam Winchester's soul and sent him out of the Cage and into the past in the hopes that he can stop this whole thing before it even starts.
Only, when Sam lands, things don't quite go so well. He's not all that sure that this isn't another torture the Cage has provided for him. He's in the past, right after Dean got out of Hell, and he's nowhere near the same man he was before. He can barely remember how to function as a human. It's going to take his family and a little help from a surprised archangel for Sam to start to recover. He doesn't have much time, though, because the apocalypse is still coming and the angels are fighting to get Lucifer free. Can one broken man manage to change the past? Or are they doomed to repeat their same mistakes over and over again?
