It's 5.40 a.m.
Last time Dean checked the clock, it was 4.45.
Before that, it was 3.20.
And before that, 2.57.
The fucking guts just don't stop gurgling.
It's the endless circle of stabs and cuts, the fire burning underneath it all. Dean tries to breathe evenly, to add the pressure and warmth with his palms wherever it hurts, curl his knees up to not to jostle his poor, aggravated muscles any further, arch his back to make more room for his bloating stomach. It all helps for a moment, coming back soon with a vengeance.

Dean Winchester, with his younger brother Sam, has ended his career as a hunter of supernatural monsters — only to have another kind of monster to tackle. The kind he has absolutely no idea how to deal with.
Thankfully he still has his brother with him. A bit of a control freak, dealing with issues of his own, but still.

Summary
Dean Winchester has never had problems in understanding what his profoundly disabled younger brother, Sam, needs or means in different situations. Sam's world is expanding though, slowly but surely, and outside Sam's protective bubble of his every day contacts — like their family friends, Sam's doctors and therapists and his personal caregiver Castiel — his language is often found indecipherable.
Sam's new communication device is about to change that.

Some home cleaning takes Dean down to a memory lane. Sam is able to bring him out of his funk though.

Summary
"Is everything alright?" Castiel asks softly, hoping that him opening the conversation would make it easier for Dean.
Dean shugs and hums with a mirthless half-smile. "I don’t know," he says, equally softly. This is clearly a conversation Sam should stay out of.
"Alright." Castiel nods, a bit unsurely. He ponders back and forth and decides against asking any more questions. He doesn’t want to seem prodding. According to his experience with the older Winchester, it might actually make him feel cornered.
Dean takes his time, gingerly sipping the hot coffee. The mug is almost empty when he looks at Castiel again.
"Dr. Lafitte is concerned for Sam," he finally says softly. "He called about the bloodworks."

Sam has a new occupational therapist and with her some new skills to learn. Castiel improves his cooking skills, and Dean has a need to talk.
