We follow along from Sam's POV as he navigates this gory new reality of his that he can only observe from a backseat in his own mind. After biding his time, Sam figures out way to gain the upper hand but isn't quite himself. The hunger and demonic urges remain, sending him on a sexual, gory and violent feeding frenzy. His obsession eventually turns to Dean, who swiftly becomes prey to this version of Sam. Can his brother bring him back to himself?
