Summary
In a modern universe, Ozai and Ursa have been divorced for almost a year. Zuko lives with Ursa, but Azula insisted on staying with Ozai. Both of the children go to therapy, but Azula just doesn't seem to make any progress, claiming that therapy is useless and she hates it. The therapist suspects the cause of this problem to be the father, so she asks for a therapy sessions with only the parents.
The session goes different than expected, revealing a much bigger problem to the therapist.

Ursa watches her husband destroy her son and imagines that Ozai knows. That Ozai knows that Zuko is Ikem's son and that's why he treats him this way. That Ozai hates him because he has reasons to hate him.
Ozai grants her request. He treats Zuko as if he were Ikem's son.
And when he does, Ursa allows herself to believe it. Because when she believes it, it becomes easier to love Ozai's son.
Or: Ursa’s marriage to Ozai was anything but easy. Sometimes, she chose the lie — because believing it was easier than loving the truth.

Holding the lump of bread like a trophy in its beak, the baby turtleduck swam back towards his mother - his battered siblings lagging behind with sore shells and necks. It held its head up high - as if it acquired the bread through a harsh and violent battle instead of latching onto the piece and whacking any competitors with his tail and shells if they came too close, and that made Ursa almost laugh out loud.
She truly loved the turtleduck pond.

It had been obvious to her that her husband was displeased and now she feared him to take that anger out on her.

The reveal in the comics that Ozai's & Ursa's marriage came about because of a prophecy makes no sense, because dammit you do not establish a great lineage through a second son unless you want a civil war. But... what if the fact that it made no sense was the whole point?

The Fire Sages bring Firelord Azulon a prophecy: That mixing the royal bloodline with that of Avatar Roku will create a mighty lineage. But there's a problem. Roku has one descendant of marriageable age, & she's already married to his second son, with a baby on the way. -
