The Institute didn’t fall quietly.
Nate gave the underground residents a chance to flee before the walls came down; there's no telling who all listened or who all stayed. Shaun was already fading— worn thin by time, sickness. Nate stayed until the end, watch his last thread to this world die... then walked away and blew the place to Kingdom Come. The Commonwealth lived.
The monster in the dark was gone.
Life continued on.
They called him a hero.Nate only felt the weight of it all. Of the choices made, the family he lost, of a fight that never really ended for him.
He kept moving, fixing things, doing what people expected. It was easier than stopping. Easier than asking what came next. Sometimes another set of footsteps matched his own. Eyes that saw too much. They never said what it was, too afraid to push.
Then the attacks began.
Progress undone overnight. A new hand pulling strings, possibly from the Institute’s ashes. And whoever it is, they have their eyes set on the Commonwealth's beloved Vault Dweller.

Vault 111 is generally considered to be a dead Vault, so he's surprised when he arrives in Sanctuary Hills just in time to witness the Sole Survivor enter the Brave New World for the first time. He doesn't know it yet, but he's about to meet the woman who would become the best damn partner he's ever had.

Her eyes were fixated on the blue crib in front of her. She never signed up for this; it was never part of the plan.
“I know!” the curled smile on his face was unsettling, the smile that was keeping a million secrets from her. “A baby, isn’t it great? Someone to carry on my legacy. He’s the luckiest boy in the world,” he sounded sickeningly happy, as if it was perfectly normal to have a baby thrusted into her arms.

Nathaniel was always a little unhinged but this was madness.
She looked down at the tiny baby in her arms and tugged him closer to her chest.
Shaun. Said the embroidery on his blanket. Pre-named and pre-made.
She watched as his mouth opened just a tiny amount and he sighed out a silent yawn, his eyes opened for a split second and the sea of blue hit her. She had green eyes herself and she looked up to see Nathaniel’s hollow brown eyes look lovingly at her.

John escapes both death and the apocalypse to walk out of Vault 111 into the world that rose from the ashes of nuclear destruction. A new, potentially better life awaits him in it, one offered by a knowledgeable and concerned third party hoping to make the Commonwealth a little less deadly.
Edit: this was originally going to be a series of oneshots exploring what Person of Interest might look like if it took place in the Fallout universe (specifically during the time of Fallout 4), but I jixed myself out of writing more by saying I was going to write more, so sadly this will remain as 1 oneshot for the foreseeable future.
