He looked across the way, hoping to see his Watson smiling and waving at him. Instead he saw a prone figure on the ground in front of the bench where he left Watson. Two women, one older and one younger, likely related, bent over the poor fellow.
“Watson!” Holmes cried, running across the street.
After his confrontation with Isadora Klein, Holmes finds Watson collapsed on the ground outside.

Summary
Holmes and I had been flatmates and companions for nearly fifteen years. It was not what one might call overly idyllic by any means, but I was quite satisfied with our lives on Baker St, and the close friendship we had formed. I would have been content with things remaining unchanged, but change came anyways, in the form of a man I had thought to never hear from again.
Watson is happy with his life with Holmes, but things are going to change after a man from Watson's past contacts him.

“Watson!” Holmes hissed under his breath, a gentle reprimand in his tone. This penetrated Watson’ warm thoughts. He knew he should release him, but the thought of letting go of Holmes’ hand was beyond him at that point. In desperation he pressed the back of Holmes’ hand to his cheek.
“Watson! What in the world--?!”
“Ah, I see that your friend has switched your drinks,” said Processor Wheatley.
Watson drinks a substance intended for Holmes, and it heightens Watson's desires.
